(English below – The University of Pavia coordinates the Horizon project for the development of an App for monitoring and assisting cancer patients) CAPABLE (CAncer PAtients Better Life Experience) è un progetto europeo finanziato nell’ambito del programma Horizon 2020, che si basa su Big Data e Intelligenza Artificiale per monitorare lo stato di salute e la qualità di vita dei pazienti malati di cancro.
Il cancro è tra le principali cause di morte nel mondo, tuttavia la percentuale di sopravvivenza sta aumentando e oggigiorno gestire bene la cura a lungo termine è la vera sfida da vincere.
I pazienti vengono curati per lo più a domicilio e accedono all’ospedale solo per visite o ricoveri urgenti o per i follow-up programmati. La criticità sta nell’aggressività dei trattamenti, che spesso comportano eventi avversi che insorgono quindi a casa e che incidono in maniera significativa sulla qualità di vita, del paziente e di chi lo assiste. I sintomi, solitamente, se non molto gravi, vengono riferiti al medico durante le visite di controllo, che hanno una frequenza di due o tre settimane o anche più. Tale monitoraggio non è ottimale, perché il paziente potrebbe anche dimenticare, sottovalutare e non riportare correttamente un sintomo insorto tra la visita precedente e quella corrente.
Il progetto CAPABLE consentirà al paziente, attraverso un’app, di inserire direttamente dati e sintomi in tempo reale. Il paziente avrà a disposizione anche uno smartwatch che raccoglierà automaticamente altri dati relatvi ad alcuni parametri vitali, oltre ad attivita fisica e qualità del sonno. La app fornirà consigli su come trattare nel migliore dei modi gli eventi avversi che si presentano al paziente, anche in accordo alle sue caratteristiche (es. età, comorbidità), dando consigli mirati su come affrontarli da casa quando possibile e quando invece è opportuno contattare il medico. Di riflesso tutti i dati e le condizioni che riguardano il paziente sono anche notificati al medico, in modo che questi abbia sempre un quadro più aggiornato, completo ed esaustivo possibile.
In aggiunta, CAPABLE non vuole solo dare un aiuto dal punto di vista della gestione delle tossicità, ma più in generale, si vuole aumentare il benessere in un’accezione più ampia, affrontando i temi dello stile di vita sano, della qualità del sonno, del benessere mentale e quindi della sfera emotiva. Pertanto si cercherà di dare dei consigli in questi campi, preparati con l’ausilio di psicologi, nutrizionisti ed esperti di terapia fisica, sempre in stretta collaborazione con l’oncologo. In questo modo si auspica che il paziente, sentendosi meno solo, non faccia scelte personali (es. diminuire o sospendere la dose del trattamento) che possono causare gravi danni. Dall’altra parte il medico non riceverà chiamate o visite da pazienti anche per sintomi molto lievi, diminuendo così gli accessi inappropriati all’ospedale.
CAPABLE unisce gli sforzi di 12 partner, in Europa e in Israele, ed è coordinato dall’Università di Pavia (Prof.ssa Silvana Quaglini). Gli studi clinici saranno svolti nel 2023 presso gli ospedali ICS Maugeri a Pavia, NKI di Amsterdam e Policlinico di Bari. Nel 2022 sarà studiato un certo numero di pazienti per valutare la loro qualità di vita e avere così un termine di confronto per capire quali saranno i benefici del progetto.

CAPABLE (CAncer PAtients Better Life Experience) is a European project funded under the Horizon 2020 program, which is based on Big Data and Artificial Intelligence to monitor the health and quality of life of cancer patients. Cancer is among the leading causes of death in the world, however the survival rate is increasing and nowadays managing long-term care is the real challenge to be overcome. Patients are mostly treated at home, and only enter the hospital for urgent visits or hospitalizations or for scheduled follow-ups. The criticality lies in the aggressiveness of the treatments, which often involve adverse events that therefore arise at home and which significantly affect the quality of life of the patient and those who care for him. Symptoms, usually, if not very serious, are reported to the doctor during check-ups, which take place every two or three weeks or even more. This monitoring is not optimal, because the patient could also forget, underestimate and not correctly report a symptom that arose between the previous visit and the current one.
The CAPABLE project will allow the patient, through an app, to directly enter data and symptoms in real time. The patient will also have a smartwatch that will automatically collect other data relating to some vital parameters, as well as physical activity and sleep quality. The app will provide advice on how to best treat the adverse events that occur to the patient, also in accordance with his/her characteristics (e.g., age, comorbidities), giving targeted advice on how to deal with them at home when possible and when it is advisable to contact the doctor. Consequently, all the data and conditions concerning the patient are also notified to the doctor, so that he always has the most updated, complete and comprehensive picture possible.
In addition, CAPABLE does not only want to help from the point of view of toxicity management, but more generally, it wants to increase well-being in a broader sense, addressing the issues of a healthy lifestyle, mental well-being and therefore of the emotional sphere. Therefore, we will try to give advice in these fields, with the help of psychologists, nutritionists and physical therapy experts, always in close collaboration with the oncologists. In this way it is likely that the patient, feeling supported, does not make personal choices (e.g., decrease or suspend the dose of treatment) that can cause serious damage to his/her health. On the other hand, the doctor will not receive calls or visits from patients even for very mild symptoms, thus reducing inappropriate access to the hospital.
CAPABLE combines the efforts of 12 partners, in Europe and Israel, and is coordinated by the University of Pavia (Prof. Silvana Quaglini). The clinical studies will be carried out in 2023 at the hospitals ICS Maugeri in Pavia, NKI in Amsterdam and Policlinico di Bari. In 2022, a certain number of patients will be studied to assess their quality of life and thus have a term of comparison to understand what benefits the project will provide.
